After booking your appointment online or through the phone, please fill out our new patient form prior to your first visit.

We are a modern clinic with the latest technology, and our waiting area is welcoming and comfortable, equipped with free Wi-Fi and an entertainment center for your kids.

Insurance and Payment Options

We also have a variety of payment options and payment plans. This includes cash, cheque, Visa, MasterCard and debit. If you have insurance coverage, we will help you understand your coverage and bill your provider directly.
